Good Faith Estimate
You have the right to receive an estimate of expected charges for your medical care if you are uninsured or are not using insurance to pay for care.
1Your right to know the expected cost
Federal law requires healthcare providers to give uninsured and self-pay patients a written Good Faith Estimate of expected charges for non-emergency items and services. You may request one before scheduling care. This can include related medical tests, prescription drugs, equipment, and other services.
Clinical care through Hälso Life is provided by Halso Life FL, LLC. The clinical practice is responsible for estimates for the care it arranges. Ask each separate provider or facility about charges for care it arranges independently.
2When an estimate is due
If you schedule care 3–9 business days in advance, your estimate is due within 1 business day after scheduling. If you schedule at least 10 business days in advance, it is due within 3 business days after scheduling. An estimate requested before scheduling is due within 3 business days of the request.
A recurring-care estimate can cover up to 12 months and must describe the expected frequency, duration, and charges. An estimate is not a contract to receive care. Keep a copy for comparison with your bill.
3How to ask
To request an estimate from Halso Life FL, LLC, contact hello@halsolife.com and ask for a Good Faith Estimate. You may also write to Halso Life FL, LLC, 1816 Clare Avenue, West Palm Beach, FL 33401.
Start with your name and a way to contact you. Do not send medical records, diagnoses, or identity documents by ordinary email; ask for a secure way to provide any additional information needed.
The membership prices and inclusions describe the plans. They do not replace your individualized estimate of clinical charges.
4If your bill is higher
If a provider or facility bills you at least $400 more than its estimated charges, you may be eligible for the federal patient-provider dispute resolution process. Generally, you must start that process within 120 calendar days of receiving the initial bill.
